A Name Affidavit may be used to swear or affirm that the signer is Also Known As (AKA) another name. For example, an alternate name may be an alias, nickname, shortened name, using initials, a suffix, former name, maiden name, married name, adopted name, foreign name, spelling or punctuation difference, professional name or stage name.
Some examples of famous names: champion boxer Muhammad Ali, AKA “The Greatest“, formerly known as (FKA) Cassius Clay. Norma Jean Baker, actress, professionally known as (PKA) Marilyn Monroe. President John Fitzgerald Kennedy, also known by his initials, JFK. Harry Potter series author Joanne Kathleen Rowling, AKA J. K. Rowling. Singer Adele Laurie Blue Adkins, known by her first name, Adele. Singer Elvis Aaron Presley, known as Elvis and “the King of Rock and Roll”. Old West legend William Frederick Cody, AKA “Buffalo Bill” (see photo).
The signer makes a sworn statement under oath or affirmation of the facts while meeting with a notary. The signer must show the notary a satisfactory form of photo identification and must sign the document in the presence of the notary. Below is a sample form for a Name Affidavit or AKA Statement.
Some versions may also include sample signatures for the name variations, known as a Signature/Name Affidavit.
