FTC Identity Theft Affidavit

As the saying goes, an ounce of prevention is worth a pound of cure.  So, take steps to protect your important computer files, confidential documents, ID cards, and credit cards.  Read the consumer protection information on the FTC website.

Do not place confidential information online with cloud storage vendors unless you are using secure, encrypted cloud storage.  We use Filen for encrypted cloud storage.

Sample Form

The FTC has a sample form for reporting ID theft.  You can download a copy, with a Colorado notary certificate attached, from our website at FTC Identity Theft Complaint Affidavit.

An affidavit is a sworn statement made before a notary or other public official authorized to administer an oath or affirmation.  You complete the affidavit form, adding the information and details for your case.  You must sign the affidavit while meeting in person with the notary.
