1. In the form below, on line 1, select the type of service received using the drop-down menu: Mobile Notary, Form I-9 Verification, or Other Service.
2. On line 2, enter your name, and the date of service.
3. On line 3, enter the type of document and any comment.
4. Then click on the Pay Now button below the form.
5. When the checkout screen appears, enter the total dollar amount and click on Update.
6. Choose to pay with a credit card, or debit card.
